Precision Meds was founded in 2023 by a team of doctors and pharmacists with a shared frustration: too many clinical pharmacists were being placed into GP practices in isolation, without the team support, supervision, or continuity that good clinical work demands.
We built Precision Meds to do it differently. Our pharmacists work at GP practices over the long term, building genuine working relationships with the GPs, nurses, and administrative teams they sit alongside. At the same time, they belong to a real team of peers — sharing cases, learning from each other, and meeting weekly for clinical governance.
It's a model that produces better clinical work, happier pharmacists, and stronger relationships with the practices we serve.

Our pharmacists are assigned to specific practices over the long term. Continuity means shared understanding of workflows, patient cohorts, and clinical priorities — and a real partnership rather than a transactional placement.

Every pharmacist attends our weekly clinical governance meeting — case discussions, complex decision review, learning topics, and team supervision. It's not an add-on; it's how we ensure consistent clinical quality across every practice we serve.
